Job Description
Under the supervision of the Nurse Manager of Hematology/Oncology Centers, the Oncology Care Coordinator acts as a liaison between patients, families, physicians, nurses, external and internal case managers, community agencies and providers and other health care professionals to implement the total continuum of patient care. M.S.W., L.I.C.S. W. required. At least five years of recent oncology care management experience required. Knowledge of institutional and community based services across the continuum of care. Negotiation skills and creative problem solving skills required. Excellent communication skills. Computer skills required. Strong interpersonal skills enabling effective team collaboration and establishment of sound professional relationships with patient and families.
