This role provides Admissions and/or Case Management visits exclusively for Home Care patients. RN licensure, preferably Bachelor's prepared in nursing or other relevant field and one or more years recent RN practice in an acute care or home care role required. Valid Driver's license and personal transportation (car) is required. Basic Life Support (BLS) CPR certification is required. Demonstrated performance maintaining a strong patient satisfaction and committment to service excellence required.
