Assumes the 24 hour management of the functions of the Case Management Department.Manages and supervises all the functional elements of a comprehensive, multidiscipline case management program, providing services across the continuum, pre-hospitalization to post discharge.Oversees and continually develops a program that satisfies state and federal regulations.Facilitates the organizational compliance that provides for patient access to an appropriate level of care, health professionals and settings to access care and service based on the patients assessedneeds.MA RN license and BSN required.Masters Degree in a health care related field or business preferred.Case Management certification preferred.3-5 years clinical acute hospital experience required.
