Job Description
RN licensure, Bachelor's degree in Nursing (BSN) and Basic Life Support (BLS) certification is required. Valid Driver's license and personal transportation (car) is required. This role provides and manages nursing care in patient homes. Certification and/or Master's degree in nursing preferred. Two years of community health or home care experience strongly preferred. Expertise in chronic disease management required as well as an ability to manage the needs of the complex patient throughout the continuum of care, in a variety of settings. Outstanding interpersonal skills and a proven ability to develop professional relationships with a Home Care multi-disciplinary team, outside care providers, patients, families and caregivers is required. Demonstrated performance maintaining a strong patient satisfaction and commitment to service excellence is required.
