Job Summary: Performs a comprehensive nursing assessment upon admission of the patient and family including physiological, social, spiritual, psychological and environmental factors as well as health history. Integrates all pertinent data gathered on assessment and identifies problems that form the basis of a plan supportive of care that maximizes the comfort and health of hospice patients and families.
Utilizes a case management model when in the role of a case manager and adheres to case management objectives when non-case manager.
