Medical Social Worker with recent Home Care experience, Master's degree (MSW) and Basic Life Support certification (CPR) required. LICSW preferred. Counsels and advises patients and their families in coping with problems related to their illness. Consults with staff to provide adequate community resources upon discharge from home care.
