Job Description
Required, Masters prepared Social Work license in the state of MA.Required a minimum of 5 or more years in an acute care hospital setting
Will spend a majority of time in the Emergency department and Maternal/Child Division while also providing support to other hospital units. Oversees the initial and ongoing assessment of each patient for appropriate discharge planning and psychosocial needs. Evaluates patient care plans for changes and monitors patient expected outcomes to facilitate appropriate discharge planning. The care coordinator does initial and on-going assessments of patients for utilization, quality of care, discharge planning, and psychosocial needs. The care coordinator works collaboratively with the health care team to ensure the best possible patient outcomes. Provides on-call services and some weekend/holiday coverage.
