Responsible for the clinical activities of the Wound Management Program (WMP) Inpatient Component. These include coordination of clinical services to provide consistent quality care for patients with wounds in the inpatient setting. Collaborates with physicians, outpatient and inpatient health care professionals, and staff of the WMP. Works within the framework of the State Practice Act and follows Healogics and Mercy Medical Center policies and procedures. Supports the goals and objectives of Mercy Medical Center and Healogics.
PRIMARY JOB RESPONSIBILITIES AND DUTIES:
Direct Patient Care Services: Provides wound assessment and continuity of care for patients in the inpatient setting in collaboration with referring and/or attending physicians, hospital staff, WMP staff and other professionals throughout the health care continuum. Works closely with the primary care physician to plan, implement, and coordinate the patients discharge needs for comprehensive skin and wound care. Demonstrates competence to instruct and monitor staff in the delivery of care as it relates to wound management. Assists in evaluating clinical staff in their clinical practice of wound care management.
Planning / Organizing / Initiating: Organizes and prioritizes clinical responsibilities and demonstrates ability to interact with health care professionals to ensure quality wound management for patients. Collects specific patient and wound data and assists the hospital in identifying cost savings related to wound care supplies and treatment modalities. Organizes and conducts wound educational programs for hospital staff. Oversees Quality Management activities ensuring integration with affiliate hospital program.
Communication: Demonstrates effective communication skills in interactions with physicians, hospital staff, patients and caregivers, referral sources. Supports and encourages harmonious working relationships and demonstrates effective team building skills.
Leadership: Demonstrates effective leadership skills to manage the day-to-day clinical activities of the inpatient component. Provides guidance in wound care and assists physicians inpatient care issues. Effectively establishes self as expert in wound care.
Health and Safety: Adheres to infection control, emergency, safety, fire and disaster plans policies, completing required initial training and updates per hospital policy. Demonstrates consistent use of Standard Precautions, proper body mechanics, and follows OSHA guidelines. Maintains a safe environment to protect patients, visitors and staff.
General Work Practices: Provides for a safe work environment, which is conducive to and complies with all state and federal guidelines. Participates in change efforts toward improvement of the work environment.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Registered Nurse with current Mass license in good standing. Five years prior clinical experience with some ambulatory care experience desirable.
BSN/WCC certification and/or case management experience required.
Minimum of two years prior supervisory or health care management required.
Possesses strong verbal and written communication skills.
Word processing and data entry skills required.
Regular and routine exposure to a variety of unpleasant physical working conditions, and frequent exposure to injury or occupational or contagious disease hazards, which require routine precautions.
Requires considerable physical effort on a consistent basis as in lifting, pulling, or pushing unyielding loads (heavy, bulky, awkward, more than 40 pounds).
Requires prolonged standing, walking, or other constant physical movements on a routine basis; or requires remaining in same position over long periods of time; or eye or ear strain.
Reports to Healogics/WMP Director with dotted line responsibilities to the Director, Nursing Systems.
Has no responsibility for the direct supervision of a staff. Provides input into evaluation of clinical staff.
