Reviews safety reports and ensures appropriate follow up is completed in a timely manner.
Facilitates the root cause analysis process in the event of a sentinel or near miss event. Coordinates review of medical information, interviews staff, debriefs all individuals as indicated, and ensures appropriate and timely follow up for identified action items.
Provides oversight of Root Cause Analysis (RCA) Reporting System-maintenance of thorough and complete documentation of root cause analysis work. Creates aggregate data reports, based on the root cause analysis documentation, as requested.
Manages CRICO RCA database
Leads patient safety efforts
Collaborates with the Risk Manager in the leadership of the Inpatient Medication Event Committee, the DFCI/Boston Childrens Hospital Safety Reporting Committee and other groups as requested.
Manages the Continuing Medical Education (CME) certifications for the Morbidity, Mortality, and Improvement Conference.
Serves as an active member of the Patient Care Assessment Committee, the Ambulatory Event Committee, the Multidisciplinary Peer Review Committee, the Continuous Readiness Committee and the Institutional Safety Committee.
Contributes to report creation for regulatory agencies as requested.
Conducts audits and chart reviews when called upon.
Collaborates with the Patient/Family Relations (PFR) staff on matters of concern voiced by patients. Covers the PFR function in the absence of the PFR Manager.
Collaborates with the Office of General Counsel and CRICO on potential or actual legal matters.
Actively promotes the use of the DFCI safety reporting system and facilitates educational sessions as indicated.
Collaborates with the Director of Risk Management and Patient/Family Relations, Senior Director of Patient Safety and Medical Director of Patient Safety on case reviews as requested.
Assists in the coordination of patient safety and risk management education and training programs.


Bachelors degree in health-related field required; clinical degree strongly preferred; Masters Degree preferred. Previous oncology clinical practice experience preferred

5+ years professional experience in clinical settings preferred

1-2 years experience in patient safety, risk management, and/or quality improvement preferred The ideal candidate will have detailed knowledge of and experience in diverse oncology practice settings (e.g., community hospitals, private practices, academic medical centers), through professional experience and/or consultative roles.
