As the Lead Community Health Worker (CHW) he/she will assume responsibility for orientation and ongoing support to other Community Health workers. Also as a member of the Patient Centered Medical Home team, the Lead CHW will assume responsibilities for the care coordination for patients. The Lead CHW works collaboratively with internal and external entities in order to remove barriers to care and ensure smooth care transitions for patients/families. The Lead CHW works with patients to address their needs by serving as the link/liaison between the patient and the rest of the care team. The Senior CHW will identify and help to close care gaps, access community services, provide support and education to the patient and family/caregivers, and engage the patient in their healthcare. The Lead Community Health Worker will support ongoing education and collaboration with all other CHWs. High School Diploma required, Associates Degree preferred. Experience as a Medical Office Coordinator or Medical Assistant in an ambulatory setting preferred. Proficiency with Microsoft Office, general knowledge of electronic medical records preferred; Basic computer skills to enter, interpret and extract medical information from electronic systems in the health care environment.
