Job Description

Maintain lines of communication with physicians to increase awareness of Agency programs and to identify customer service issues through regularly scheduled face-to-face and telephone contacts with the individual MD or designee.

Provides education/presentations to community partners, ALFS, SNFS, MDs, and Hospitals as requested and arranged by the Provider Relations Team.

Maintains relationships with facility discharge planners and community entities to increase awareness of Agency programs and to identify areas for improvement in Agency service delivery that impact customer satisfaction

Sustains lines of communication with Director Provider and Community Relations to ensure coordination of customer satisfaction efforts. Attends regularly scheduled meetings with Directors to review goals and productivity expectations.

Serve as Liaison with physicians offices, skilled nursing facilities, providers and hospitals

Determine referral appropriateness for VNA service lines and obtain referral from physicians office, skilled nursing facilities, off Cape providers and hospitals

Communicates issues and provides contact information for appropriate party to provide feedback to the referral source and does follow up with referral source to assure resolution

Able to efficiently utilize and analyze trending reports to assure appropriate management to schedule referral sources

Participates in Agency marketing activities as assigned including presentations to physicians offices, skilled nursing facilities, off Cape providers and hospitals

Participates in customer service education of Agency staff in alignment with Agency and CCHC policies and procedures

Other duties as assigned

Qualifications

Registered Nurse currently licensed in Massachusetts preferred. Five (5) years recent clinical experience, including two (2) years of recent community health experience required, home health preferred. Previous experience in Sales or Marketing required. Computer experience and knowledge of Information Systems required. Customer Relations experience preferred.
